House Democratic leaders unveiled a comprehensive health care reform plan on June 19, including a proposed public health insurance program that would compete with private insurers to lower costs. The proposal would create a Health Insurance Exchange to provide a functional marketplace for individuals and small employers to comparison shop among private and public insurers. Few details of how the plan would be paid for were announced, and lawmakers said they were awaiting estimates from the Congressional Budget Office.

Beyond the changes to the larger U.S. health insurance system, the 850-page bill includes about 550 pages of changes and additions to the Medicare and Medicaid programs. These changes fall into seven main categories: (1) improving health care value under Medicare Parts A, B, C, and D; (2) Medicare beneficiary improvements; (3) promoting primary care, mental health services, and coordinated care; (4) quality; (5) Medicare graduate medical education; (6) program integrity; and (7) Medicaid and State Children's Health Insurance Program (SCHIP) changes.
